Care Management
Care Management, Dietitian and Behavioral Health visits are done in our office and/or by phone calls. We answer questions about your health, medications and diet.
We enhance communication with your physician and make referrals to other resources in the community for additional support for you and your family. We offer support and tools to address behavioral needs.

Rapid SARS-CoV-2 TESTING
We offer rapid COVID-19 screening tests to our asymptomatic patients and families. Results within 15 minutes. An appointment is required.
Call 231-935-0850. Insurance’s will not be billed; payment is expected at time of service.
We will test our symptomatic patients also following a virtual appointment with your doctor. Insurance's may be billed in this instance.
